1. (1) A license to practice dentistry, dental hygiene, or dental therapy shall be issued to Active-Duty Members of the Uniformed Services or their spouse or domestic partner when the following requirements are met:

   1. (a)  A completed application and payment of fee is received by the Board; and
   2. (b) Submission of a copy of the military orders assigning the active-duty member to an assignment in Oregon; and
   3. (c) The spouse holds a current license in another state to practice dentistry, dental hygiene, or dental therapy at the level of application; and

   4. (d) The license is in good standing and verified as active and current through processes defined by the Board; and

      The license shall remain active for the duration of the above-mentioned military orders.

2. (2) Each biennium, the licensee shall submit to the Board a Biennial Military Status Confirmation Form. The confirmation form shall include the following:

   1. (a) Licensee's full name:
   2. (b) Licensee’s mailing address;
   3. (c) Licensee’s business address including street and number. If the licensee has no business address, licensee’s home address including street and number;
   4. (d) Licensee’s business telephone number. If the licensee has no business telephone number, licensee’s home telephone number;
   5. (e) Licensee’s employer or person with whom the licensee is on contract;
   6. (f) Licensee’s assumed business name;
   7. (g) Licensee’s type of practice or employment;
   8. (h) A statement that the licensee has met the continuing educational requirements for their specific license renewal set forth in OAR 818-021-0060 or OAR 818-021- 0070 or OAR 818-021-0076;
   9. (i) Identity of all jurisdictions in which the licensee has practiced during the two past years;
   10. (j)  A statement that the licensee has not been disciplined by any licensing board of any other jurisdiction or convicted of a crime; and
   11. (k) Confirmation of current active-duty status of service member.
