1. (1) Applicants and authorization holders are subject to provisions of OAR 331-010-0010 and 331-010-0020 regarding payment of fees, penalties and charges.

2. (2) Fees established by the Office, in consultation with the Board, are as follows:

   1. (a) Application:

      1. (A) Practitioner certificate: $30 per field of practice.
      2. (B) Practitioner certificate by reciprocity: $100 per field of practice.
      3. (C) Independent contractor registration: $70.
      4. (D) Freelance authorization: $35.
      5. (E) Facility license: $140.
      6. (F) Temporary facility permit: $70.
      7. (G) Demonstration permit: $25.

   2. (b) Examination:

      1. (A) Oregon laws & rules: $45.
      2. (B) Barbering: $45.
      3. (C) Hair design: $45.
      4. (D) Esthetics: $45.
      5. (E) Nail technology: $45.
      6. (F) Natural Hair Care: $45.

   3. (c) Original issuance of authorization to practice:

      1. (A) Practitioner certificate: $35 for two years.
      2. (B) Practitioner certificate by reciprocity: $65 for two years.
      3. (C) Independent contractor registration: $140 for one year.
      4. (D) Freelance authorization: $140 for one year.
      5. (E) Facility license: $155 for one year.

   4. (d) Permits:

      1. (A) Temporary facility: $140.
      2. (B) Demonstration: $50.

   5. (e) Renewal of authorization to practice:

      1. (A) Practitioner certificate: $65 for two years. Fee applies to each two-year renewal cycle even while in inactive status for up to three years.
      2. (B) Independent contractor registration: $140 for one year.
      3. (C) Freelance authorization: $140 for one year.
      4. (D) Facility license: $155 for one year. Fee applies to each renewal cycle even while in inactive status.

   6. (f) Other administrative fees:

      1. (A) Delinquency fee: $50 for each year in inactive status up to three years.
      2. (B) Replacement of freelance authorization, certificate, license or registration, including name change: $35.
      3. (C) Duplicate freelance authorization, certificate, license or registration document: $25 per copy with maximum of three.
      4. (D) Affidavit of licensure: $50.
      5. (E) An additional $25 administrative processing fee will be assessed if a NSF or non-negotiable instrument is received for payment of fees, penalties and charges. Refer to OAR 331-010-0010.
      6. (F) Information packets: $10.
