1. (1) A person applying for a license as a lead abatement contractor must submit the following:

   1. (a) Completed application on a form provided by the board;
   2. (b) Proof that the person is licensed by the board as a construction contractor;
   3. (c) The fee established in OAR 812-007-0260;
   4. (d) Proof that the applicant is owned by or employs one or more persons who are licensed lead supervisor(s); and
   5. (e) Proof that OHA certified the person as qualified to perform abatement.

2. (2) A person applying for a license as a lead inspection contractor must submit the following:

   1. (a) Completed application on a form provided by the board;
   2. (b) Proof that the person is licensed by the board as a construction contractor;
   3. (c) The fee established in OAR 812-007-0260;
   4. (d) Proof that the applicant is owned by or employs one or more persons who are licensed lead inspector(s) or licensed lead risk assessor(s); and
   5. (e) Proof that OHA certified the person as qualified to perform inspection or risk assessment.
