1. (1) An individual applying for a license as a lead assessor must submit the following:

   1. (a) Completed application on a form provided by the board;
   2. (b) The fee established in OAR 812-007-0160; and
   3. (c) Proof that OHA certified the individual as a certified risk assessor.
   4. (d) Copy of OHA photo identification badge.

2. (2) An individual applying for a license as a lead inspector must submit the following:

   1. (a) Completed application on a form provided by the board;
   2. (b) The fee established in OAR 812-007-0160; and
   3. (c) Proof that OHA certified the individual as a certified inspector.
   4. (d) Copy of OHA photo identification badge.

3. (3) An individual applying for a license as a lead supervisor must submit the following:

   1. (a) Completed application on a form provided by the board;
   2. (b) The fee established in OAR 812-007-0160; and
   3. (c) Proof that OHA certified the individual as a certified supervisor.
   4. (d) Copy of OHA photo identification badge.

4. (4) An individual applying for a license as a lead worker must submit the following:

   1. (a) Completed application on a form provided by the board;
   2. (b) The fee established in OAR 812-007-0160; and
   3. (c) Proof that OHA certified the individual as a certified abatement worker.
   4. (d) Copy of OHA photo identification badge.
