1. (1) A person may apply for a replacement ATV Safety Education Card from OPRD if:

   1. (a) They legally change their name;
   2. (b) The card is lost, stolen or destroyed;
   3. (c) Misinformation is printed on the card; or
   4. (d) The card has a printing error or physical defect.
2. (2) To obtain a replacement card, an applicant must provide OPRD with a completed application form provided by OPRD which includes an affidavit signed by the applicant stating the circumstances that led to the need for replacement of the original card.
