1. (1) Application for a permit may be made in person or online. The completed permit may be mailed or transmitted electronically for pickup by the applicant.

2. (2) The applicant must provide:

   1. (a) Permittee name and address;
   2. (b) Power vehicle year and make;
   3. (c) Power vehicle identification number (also known as a VIN); and
   4. (d) Power vehicle unit number, if one has been assigned by the permittee.
3. (3) Permits will not be issued when an application is incomplete.
4. (4) For purposes of this rule, “power vehicle” means the vehicle that provides motive power.
