1. (1) Application for a permit may be made in person or online. The completed permit may be mailed, transmitted electronically, or made available online for the applicant.

2. (2) The applicant must provide:

   1. (a) Identification of food process plant where the movement will originate.
   2. (b) The highways to be traveled.
   3. (c) The highway mile point or other identifiable geographical point where the movement will leave the highway.
   4. (d) Permittee name and address;
   5. (e) Power vehicle year and make;
   6. (f) Power vehicle identification number (also known as a VIN); and
   7. (g) Power vehicle unit number, if one has been assigned by the permittee.
3. (3) Permits will not be issued when an application is incomplete.
