1. (1) Application for permits may be made in person or online. The completed permit may be mailed, transmitted electronically, or made available online for the applicant.

2. (2) Routine information such as permittee name, address and vehicle identification must be included for the application. The applicant must provide:

   1. (a) Permittee name and address;
   2. (b) Power vehicle year and make;
   3. (c) Power vehicle identification number (also known as a VIN); and
   4. (d) Power vehicle unit number, if one has been assigned by the permittee.
3. (3) Permits will not be issued when an application is incomplete.
4. (4) Carriers who have unsatisfactory safety ratings from the U.S. Department of Transportation or the Oregon Department of Transportation, Commerce and Compliance Division are not eligible for permits under these rules.
