The applicant shall submit to the Department a consolidated application that includes but is not limited to the following sections:

1. (1) General Information;
2. (2) Existing Environment – Baseline Data;
3. (3) Operating Plan;
4. (4) Reclamation and Closure Plan;
5. (5) Alternatives Analysis.
