1. (1) When the individual refuses to participate in a required activity, documentation of that refusal shall be provided within 10 calendar days and shall at a minimum contain:

   1. (a) Name of the individual;
   2. (b) Description of the refusal including reason for refusal;
   3. (c) Signature of the individual or individual’s representative;
   4. (d) Signature of Program personnel documenting the individual’s refusal;
   5. (e) Date of signatures; and
   6. (f) Date and method of transmittal of documentation of refusal to the individual.
2. (2) The Program shall retain a copy of all documentation in a manner consistent with the Program’s case management system.
