The Notice of Proposed Action must include the following:
- (1) The date the Notice of Proposed Action is mailed;
- (2) A short and plain statement about the action the Program intends to take, including citations to the regulations, rules, statutes or policies involved;
- (3) Statement about the reason for the action;
- (4) Statement indicating whether and under what circumstances an order by default may be entered;
- (5) A description of the procedure and time to request a hearing.
- (6) Statement of the authority of the Program; and,
- (7) The information provided in the Vocational Rehabilitation Dispute Resolution Rights.
