Or. Admin. R. 581-021-0665 – Submission of Coordinator Contact Information

(1) In a time and manner required by the Oregon Department of Education, each district shall provide the department with the contact information of:
1. (a) Each civil rights coordinator designated by or for the district pursuant to OAR 581-021-0655; and
2. (b) Each coordinator designated by the district pursuant to Title IX of the Education Amendments of 1972, Section 504 of the Rehabilitation Act of 1973, and Title II of the Americans with Disabilities Act of 1990.
(2) The contact information required by subsection (1) of this rule shall include, at a minimum, each coordinator’s name or title, work address, email address, and telephone number.
