Participating institutions must make written requests to the Commission for authorization to place student participants in specific governmental and community service organizations. Such requests shall include:

1. (1) Full name and address of the organization;
2. (2) For non-governmental service organizations, information about the status of the organization under Section 501(c)(3) of the Internal Revenue Code.
3. (3) A description of the services which the student participant will perform.
