1. (1) A party who files a document with the Hearings Division shall include with the document a proof of service which states the date and manner of compliance with OAR 438-085-0541, including the address at which the other party was served.

2. (2) Proof of service may be made by:

   1. (a) Affidavit of service;
   2. (b) Written statement made upon the document filed, and signed by the party or representative making the statement; or
   3. (c) Letter of transmittal.
