Conditions for use of worksite modification assistance by the worker are as follows:

1. (1) Modifications may be provided for requests received within three years from the date of hire;
2. (2) A worker may use worksite modification assistance as often as necessary but only once per employer, with each use is limited to $35,000; and
3. (3) A worker can use a second worksite modification with the same employer for a new job if the majority of the job duties have changed.
