Conditions for use of employment purchases by the employer at injury are:

1. (1) The employer must submit to the division a completed Form 2350, “Preferred Worker Employment Purchase Agreement,” listing items that are required of the worker to perform the job for which the worker is employed; and
2. (2) The employer may use each employment purchase category once per eligibility period.
