1. (1) A program provider must prepare and maintain an accurate individual written record of personal property that has significant or monetary value to each child as determined by a documented ISP team or parent or guardian decision.

2. (2) The record must include:

   1. (a) The description and identifying number, if any;
   2. (b) Date of inclusion in the record;
   3. (c) Date and reason for removal from the record;
   4. (d) Signature of in-residence caregiver or staff making each entry; and
   5. (e) A signed and dated annual review of the record for accuracy.
