1. (1) Discharge Summary Required. A discharge summary shall be completed for each resident before discharge.

2. (2) Contents. The discharge summary shall include:

   1. (a) A recapitulation of the resident’s stay;
   2. (b) A final summary of the resident’s status, including the most recent nursing assessment as defined in OAR 411-086-0060; and
   3. (c) A post-discharge plan of care developed in accordance with OAR 411-086-0060 which will assist the resident to adjust to his/her new living environment. A post-discharge plan is not required when the resident is discharged to acute care or to the morgue.
