1. (1) Providers of chore services must have a distinct Medicaid provider number before the work begins.
2. (2) No monetary funds shall be released to the provider until the work is finished and meets the specifications of the chore service agreement.
3. (3) Providers must submit the final invoice for payment within one year of the date of service.
4. (4) Providers must supply a revised bid when requested by designated Central Office staff.
