1. (1) Table 122-0420, Hospital Bed Accessories Procedure codes — Trapeze Bars:

   1. (a) Indications and coverage: Trapeze bars are indicated when a client needs this device to sit up because of respiratory condition, to change body position for other medical reasons, or to get in or out of bed;

   2. (b) The Division may consider coverage for bariatric trapeze bars only coded as E1399 (durable medical equipment, miscellaneous) for a client residing in a nursing facility), subject to service limitations of Division rules, only when the following requirements are met:

      1. (A) The client meets the conditions of coverage as specified in this rule; and
      2. (B) The bariatric trapeze bar has been assigned code E1399 by the Medicare Pricing, Data Analysis and Coding (PDAC) contractor;
      3. (C) Supporting documentation has been submitted to the appropriate authorizing authority for prior authorization;
   3. (c) Documentation of medical appropriateness which has been reviewed and signed by the prescribing practitioner must be kept on file by the DME provider;
