(1) Applicants and registrants are subject to the provisions of OAR 331-010-0010 and 331-010-0020 regarding the payment of fees, penalties and charges.
(2) Fees established by the Health Licensing Office are as follows:
1. (a) Application:
  1. (A) Registration — environmental health specialist: $150.
  2. (B) Registration — waste water specialist: $150.
  3. (C) Registration by reciprocity: $150.
  4. (D) Trainee registration — environmental health specialist: $150.
  5. (E) Trainee registration — waste water specialist: $150.
2. (b) Examinations:
  1. (A) Oregon Environmental Health Specialist Trainee Laws and Rules exam: $50.
  2. (B) Written — waste water specialist: $250.
3. (c) Original issuance of registration (including by reciprocity):
  1. (A) Environmental health specialist: $150 for one year.
  2. (B) Waste water specialist: $150 for one year.
  3. (C) Trainee registration — environmental health specialist: $150 for one year.
  4. (D) Trainee registration — waste water specialist: $150 for one year.
4. (d) Renewal of registration:
  1. (A) Environmental health specialist: $150 for one year.
  2. (B) Waste water specialist: $150 for one year.
  3. (C) Environmental health specialist trainee extension: $150 (one-year increments)
  4. (D) Waste water specialist trainee extension: $150 (one-year increments)
5. (e) Other administrative fees:
  1. (A) Late renewal of registration: $40.
  2. (B) Replacement registration, including name change: $25.
  3. (C) Affidavit of Licensure: $50.
  4. (D) Administrative processing fee: $25.
