The Board shall report to the Secretary of Health and Human Services in the manner prescribed by the Secretary the following information:

1. (1) Any disciplinary action taken by the Board as the result of a formal proceeding concluded against a speech-language pathologist or audiologist.
2. (2) Any dismissal or closure of the proceedings by reason of the speech-language pathologist or audiologist leaving the state.
3. (3) Any other loss of the license of the speech-language pathologist or audiologist, whether by operation of law, voluntary surrender, or otherwise.
