Or. Admin. R. 333-250-0230 – Expiration and Renewal of License

(1) Each license to operate an ambulance service:
1. (a) Shall be valid until June 30 of each year, unless sooner revoked or suspended. The initial licensing period may not exceed 15 months; and
2. (b) Shall expire on June 30 of the following year, if a license is applied for and issued between April 1 and June 30.
(2) If a license renewal is desired, the licensed ambulance service shall make application and pay the appropriate fee at least 30 days prior to the expiration date in a manner prescribed by the Oregon Health Authority.
