Except as provided in OAR 333-116-0740, the licensee must require the authorized user using only strontium-90 for ophthalmic radiotherapy to be a physician who:

1. (1) Is an authorized user under OAR 333-116-0690 or equivalent U.S. Nuclear Regulatory Commission or Agreement State requirements; or

2. (2) Has completed 24 hours of classroom and laboratory training applicable to the medical use of strontium-90 for ophthalmic radiotherapy.

   1. (a) The training must include:

      1. (A) Radiation physics and instrumentation;
      2. (B) Radiation protection;
      3. (C) Mathematics pertaining to the use and measurement of radioactivity; and
      4. (D) Radiation biology; and

   2. (b) Supervised clinical training in ophthalmic radiotherapy under the supervision of an authorized user at a medical institution that includes the use of strontium-90 for the ophthalmic treatment of five individuals. This supervised clinical training must involve:

      1. (A) Examination of each individual to be treated;
      2. (B) Calculation of the dose to be administered;
      3. (C) Administration of the dose;
      4. (D) Follow up and review of each individual's case history; and
      5. (E) Has obtained written attestation, signed by a preceptor authorized user who meets the requirements in OAR 333-116-0690, 333-116-0700, 333-116-0740, or equivalent U.S. Nuclear Regulatory Commission or Agreement State requirements, that the individual has satisfactorily completed the requirements in section (2) of this rule and is able to independently fulfill the radiation safety related duties as an authorized user of strontium-90 for ophthalmic use.
