1. (1) If a parent believes an exclusion order is in error, the parent may contact the local health department and request that the local health department review and re-check the information to determine the accuracy of the exclusion order.
2. (2) A local health department must review and re-check a child's immunization records upon receipt of a request by a parent.
3. (3) If the local health department finds an exclusion order was issued in error the exclusion order must be rescinded in accordance with OAR 333-050-0310.
