1. (1) Pursuant to ORS 443.075, an agency must have an order for a plan of care from a physician or allowed practitioner for at least one of the following services and supplies for each patient:

   1. (a) Skilled nursing;
   2. (b) Physical therapy services;
   3. (c) Occupational therapy services;
   4. (d) Speech therapy services;
   5. (e) Medical social services;
   6. (f) Home health aide services; or
   7. (g) Medical supplies, other than drugs and biologicals, and medical devices;
2. (2) The plan of care shall be reviewed by the physician or allowed practitioner in accordance with CMS Conditions of Participation.
