1. (1) The following documents shall be considered by the state registrar to meet the documentation requirements for delayed registration of live birth described in OAR 333-011-0230 if the documents contain sufficient information to identify the proposed registrant as the subject of the document and support the facts of birth as reported by the applicant. Documents may include:

   1. (a) Licensed medical facility records of the proposed registrant;
   2. (b) A certified copy of an accepted application for a Social Security card for the proposed registrant;
   3. (c) The proposed registrant’s mother’s medical record if the proposed registrant’s name, date of birth and place of birth are included in the record;
   4. (d) A certified copy of school records of the proposed registrant;
   5. (e) A certified copy of census records;
   6. (f) Military records of the proposed registrant;
   7. (g) A certified copy of marriage record;
   8. (h) A certified copy of birth record of the proposed registrant’s child;
   9. (i) Voter registration records for the proposed registrant; or
   10. (j) Other official documents acceptable to the state registrar.
2. (2) All documents submitted in support of an application for delayed registration of live birth must be original documents or certified copies of original documents. Certified copies must be authenticated by the official custodian of the record.
