In order to be eligible for the ScreenWise BCC a client must meet the following eligibility criteria:

1. (1) Have an income based on family size that is at or below 250 percent of the Federal Poverty Level at the time of enrollment; and
2. (2) Reside or declare an intent to reside in Oregon; and
3. (3) Have no health insurance or be underinsured; and

4. (4) Meet one of the following criteria:

   1. (a) Be a woman age 21 or over for clinically recommended breast and cervical cancer screening and diagnostic services; or
   2. (b) Be a man of any age who is displaying signs or symptoms that may indicate breast cancer.
