1. (1) The Authority may deny a new or renewal patient application if:

   1. (a) The applicant or patient did not provide the information required to be submitted in OAR 333-008-0020;
   2. (b) The Authority determines that the information provided was falsified;
   3. (c) The Authority determines that the applicant or patient violated a provision of ORS 475C.770 to 475C.919, ORS 475C.574, ORS 475C.640, OAR chapter 333, division 7, these rules, or an ordinance adopted pursuant to ORS 475C.897.
2. (2) An individual whose application is denied may not reapply for at least six months from the date of the denial unless otherwise authorized by the Authority.
