An individual applying for a temporary license must:

1. (1) Meet the requirements of OAR 331 Division 30;

2. (2) Provide proof of having completed:

   1. (a) The 520 hours of practical training as described in 331-640-0005 (1), or the equivalent as evaluated by the Office; and
   2. (b) the IHS’s Distance Learning Program for Professionals in Hearing Health Sciences.
3. (3) Submit a completed application form prescribed by the Office, which must contain the information listed in OAR 331-030-0000 and be accompanied by payment of all required fees.
