In addition to the requirements in OAR 331-480-0005, if an applicant is licensed or certified in another state, they must:

1. (1) Be in good standing in every state in which they are licensed or certified, and
2. (2) Ensure that the Office receives an affidavit of licensure pursuant to OAR 331-030-0040 from every state in which they are authorized to practice as a lactation consultant.
