1. (1) Applicants and licensees are subject to the provisions of OAR 331-010-0010 and 331-010-0020 regarding the payment of fees, penalties and charges.

2. (2) Fees established by Office are:

   1. (a) Application:

      1. (A) License: $100.
      2. (B) License by reciprocity: $150.
   2. (b) Examination — Oregon laws and rules: $50.
   3. (c) Original issuance of license (including by reciprocity): $125 for one year.
   4. (d) Permits and waivers: $150.
   5. (e) Renewal of license: $125 for one year.
   6. (f) Delinquent (late) renewal of license: $40 per year, up to three years.
   7. (g) Reinstatement: $150.
   8. (h) Replacement of license, including name change: $25.
   9. (i) Duplicate license: $25 per copy with maximum of three.
   10. (j) Affidavit of licensure: $50.
   11. (k) An additional $25 administrative processing fee will be assessed if a NSF or non-negotiable instrument is received for payment of fees, penalties and charges. Refer to OAR 331-010-0010.
3. (3) Individuals who renew online on or after April 1, 2022, will receive a $60 discount.
