An individual applying for an athletic trainer license must:

1. (1) Meet the requirements of OAR 331 Division 30.
2. (2) Submit a completed application form prescribed by the Office, which must contain the information listed in OAR 331-030-0000,

3. (3) Submit:

   1. (a) Official documentation of having passed the Board-approved BOC examination; and
   2. (b) Proof of being at least 18 years old; and

   3. (c) Evidence of current certification in cardiopulmonary resuscitation (CPR) including:

      1. (A) Adult, child and infant CPR;
      2. (B) Adult, child and infant automated external defibrillator (AED);
      3. (C) Two person CPR;
      4. (D) Barrier devices (e.g., pocket mask, bag valve mask).
4. (4) Pay fees.
