Inmates subject to the involuntary administration process shall be given written notice at least 24 hours in advance of the hearing by the independent examining physician. The notice shall include:

1. (1) The date and time of the hearing;
2. (2) The inmate’s diagnosis;
3. (3) A statement of the clinical basis for the diagnosis;
4. (4) A statement of the clinical basis for the determination that involuntary administration of psychotropic medications is in the inmate’s medical interest; and
5. (5) An explanation of the inmate’s rights.
