1. (1) A verbatim recording of the hearing shall be made. Upon completion of the hearing, the hearings officer shall prepare a written hearing record of the decision and the supporting reasons within seven days for transmittal to the Institutions Administrator. The hearing decision shall be documented in the Correctional Information System.

2. (2) The record of the formal hearing shall include:

   1. (a) Request for Administrative Housing form (CD 1482);
   2. (b) Notice of hearing and copy of this rule (OAR 291-046);
   3. (c) Supporting documentation; and
   4. (d) Preliminary Order, Conclusion, and Recommendation of hearings officer.
