The Future Disposition Hearing Packet shall contain:

1. (1) Institution face sheet;
2. (2) Revocation recommendation;
3. (3) Final order of revocation;
4. (4) Administrative action sheet;
5. (5) Revocation hearing findings;
6. (6) Board Action Form ordering parole or Board Action Form ordering post-prison supervision conditions;
7. (7) Disciplinary report, when extension is recommended;
8. (8) Recommendation regarding statutory and meritorious goodtime;
9. (9) Correspondence;
10. (10) Statements of imprisonment for violation;
11. (11) Face sheet from old parole analysis report or comparable report; and
12. (12) Adult in custody’s Notice of Rights and Board of Parole & Post-Prison Supervision Procedures.
