The Misconduct Board Review Packet shall contain:

1. (1) Institution face sheet;
2. (2) All Board Action Forms since the prison term hearing;
3. (3) Psychological evaluations;
4. (4) Disciplinary report from the institution;
5. (5) Correspondence;
6. (6) Field parole analysis report;
7. (7) Court orders;
8. (8) PSI, PSR, PAR or document of similar content;
9. (9) DOC hearings officer’s findings and recommendations;
10. (10) Inmate’s Notice of Rights and Board of Parole & Post-Prison Supervision Procedures.
