The Personal Review Packet shall contain:
- (1) Institution face sheet;
- (2) All Board Action Forms since the prison term hearing;
- (3) Psychological evaluations (last 6 months);
- (4) Recommendation to reset the release date to an earlier date;
- (5) Correspondence;
- (6) Field parole analysis report or report of similar content; and
- (7) Court orders.
