The Secretary of State hereby adopts by reference and designates the following forms for filing a petition for incorporation of unincorporated territory as a city:

1. (1) SEL 701, Prospective Petition for Incorporation, which designates not more than three chief petitioners;
2. (2) SEL 702, Petition for Incorporation of a City Signature Sheet;

3. (3) SEL 702a, Petition for Incorporation of a City Signature Sheet, to be used when the territory proposed to be incorporated is within the jurisdiction of a local government boundary commission.
