1. (1) Definition

   1. (a) “Document File” A document file includes:

      1. (A) Documents initially filed for corporations, limited partnerships, limited liability companies, limited liability partnerships, business trusts, and trade and service marks; and
      2. (B) Supplementary documents for corporations, limited partnerships, limited liability companies, limited liability partnerships, business trusts, and trade and service marks; and
      3. (C) Assumed Business Name applications, amendments, withdrawals, cancellations, and renewals.

   2. (b) “Single Documents” Examples of single documents include:

      1. (A) Annual Reports;
      2. (B) Change of Registered Agent.
2. (2) Certified paper copies of a document file — $15 per file.
3. (3) Regular paper copies of a document file — $5 per file.
4. (4) Certified paper copies of single documents — $15 per document.
5. (5) Regular paper copies of single documents — $5 per document.
6. (6) Regular paper copies of a document at the time of filing —$5.
7. (7) Certificate of Existence — $10.
8. (8) Certificate of Record — $10.
9. (9) Certificate of No Record — $10.
10. (10) Regular paper copies of inactive corporate records — $10.
11. (11) Decorative Certificate of Incorporation/Organization/Registration — $20.
