1. (1) A raffle licensee shall file an annual report with the Department of Justice no later than 60 days after the end of the license year. The report shall be on a form prescribed by the Department. The report shall include the following information:

   1. (a) The number of raffle games held during the license year;
   2. (b) The date of each drawing;
   3. (c) The total sales of each game;
   4. (d) The total expenses relating to the conduct of each raffle game;
   5. (e) The total amount of cash prizes and the total cost to the licensee of all noncash prizes awarded;
   6. (f) The total expenses of all games expressed as a percentage of the total raffle handle; and
   7. (g) The net income from raffle games.
2. (2) All raffle reports shall be signed by a responsible official of the organization.
