An Applicant for the formation of a port must:

1. (1) Send the following, prior to any official filing with the appropriate county, to Attn: Port Formation Application, Oregon Business Development Department, State Lands Building Suite 200, 775 Summer Street NE, Salem, OR 97301-1280:

   1. (a) A sample of the petition or order to be used for purposes of ORS 198.705 to 198.955, as applicable;
   2. (b) A letter formally requesting formation of the proposed Port;
   3. (c) A legal description and map of the port boundaries; and
2. (2) Materials requested in OAR 123-020-0015 to 123-020-0035.
