1. (a) Application after application is voided for failure to complete the supervised experience requirement within sixty (60) months as described in Subchapter 86:20-13-2(e)(3) shall include the following documents:

   1. (1) Application form,
   2. (2) Official transcript(s),
   3. (3) Application Fee,
   4. (4) Completed criminal background check.

2. (b) Applicant shall provide a passing score on:

   1. (1) The Practitioner's Examination of Psychological Knowledge or another equivalent examination as determined by the Board; and
   2. (2) The LBP State Standards Test.
3. (c) Exam results accrued prior to date of this application shall not be considered.
4. (d) Internship Documentation Form may carry over to a new application
5. (e) All previously submitted and approved Supervised Experience shall be voided with prior application and shall not carry over to a new application.
