Materials required of LBP applicants

In addition to the application form and fee, an applicant for LBP licensure shall file the following completed materials with the Board prior to taking the examination:

(1) Internship/practicum Documentation Form;
(2) Official transcript(s); and
(3) Completed criminal background check.
