- (a) Certificate. The license issued by the Board shall contain the licensee's name, license number, and date of issuance.
- (b) Signature. Official licenses shall be signed by the Chair of the Board and be affixed with the seal of the Board.
- (c) Property of the Board. All licenses issued by the Board shall remain the property of the Board and must be surrendered on demand.
- (d) Notification. After having fulfilled all requirements for licensure, the Board shall send notification to the licensee of qualification for licensure; and when the license fee is received and the Board approves the candidate for licensure, the license certificate shall be issued.
