1. (a) The Supervision Agreement shall include the following:

   1. (1) Supervisor and supervisee agree to terms set forth for the accrual of supervised experience; and
   2. (2) A reproduction of the regulation regarding supervised experience.

2. (b) Evaluation of Supervised Experience document shall include the following:

   1. (1) Identifying information;
   2. (2) Time,place and duration of supervised experience;
   3. (3) Percentage of time spent in different counseling activities;
   4. (4) Supervisor's rating of professional activity;
   5. (5) Supervisor's comment section; and
   6. (6) Record of supervised experience.
