If the licensee fails to renew the license by the expiration date, the Board shall send a notification which shall include:

1. (1) Suspension of the license and forfeiture of rights and privileges granted by the license, and,
2. (2) The LPC has the right to renew the license by payment of the renewal fee and the late renewal fee and fulfillment of all other renewal requirements for up to one (1) year following the suspension of the license.
