A report to the Commissioner is required when there is a change in the chief executive officer. Such report shall include:

1. (1) A biographical sketch of individual involved;
2. (2) A statement of the past and present business affiliations of the proposed Chief Executive Officer;
3. (3) A financial statement dated within ninety (90) days. This information shall be confidential.
