1. (a) An own risk guaranty, on a form approved by the Commission, must be submitted for any additional named insured included on the permit.
2. (b) If the individual own risk employer has a parent company that is not included on the permit, and the employer is relying on its parent company's financial statements to apply, then an own risk guaranty, on a form approved by the Commission, must be submitted from the parent company for its subsidiary.
