1. (a) Fee Schedule.

   1. (1) Examination. The following fees shall be assessed for licensure and examination of veterinarians:

      1. (A) North American Veterinary Licensing Examination - At Cost + $185.00
      2. (B) Oklahoma State Examination - $150.00

   2. (2) Licensure. The following fees shall be assessed for licensure of veterinarians:

      1. (A) Annual renewal (prior to June 30) - $225.00
      2. (B) Annual renewal Faculty License (prior to July 15) - $175.00
      3. (C) Reactivation fee (between July 1 and August 29 as referenced in the Act 698.10a(D)) - $275.00
      4. (D) Reinstatement fee (after August 29) - $625.00
      5. (E) Licensure by Endorsement - $625.00
      6. (F) Faculty License - $125.00
   3. (3) Duplication or modification of license. A fee of $50.00 shall be assessed for duplication or modification of a veterinary license.

   4. (4) Supervised Doctor of Veterinary Medicine. The following fees shall be assessed for certification as a supervised doctor of veterinary medicine:

      1. (A) Original Certificate - $125.00
      2. (B) Extension - $100.00
      3. (C) Transfer - $50.00

   5. (5) Miscellaneous fees. The following miscellaneous fees shall be assessed by the Board.

      1. (A) Certification of scores - $40.00
      2. (B) Verification of license - $20.00
      3. (C) Duplication of proof of renewal of license - $10.00
      4. (D) Certification of public records (per page) - $1.00
      5. (E) Duplication of public records (per page) - $.25
      6. (F) Transcript of public records recorded (per page) - At Cost
      7. (G) Issuance of subpoena - $.25
      8. (H) Returned check processing fee - $35.00

      9. (I) Probation fees:

         1. (i) $50.00/month, unless otherwise modified by the Board or the Secretary/Treasurer
         2. (ii) Investigation/Prosecution - at cost (Non-payment of investigation, prosecution or probation costs or fees within 30 days of billing may be grounds for imposition of additional sanctions by the Board).
      10. (J) Declaratory ruling - $300.00 plus costs.
      11. (K) Continuing education extension fee - $100.00 (active military exempt)

   6. (6) Registered Veterinary technician fees. The following registered veterinary technician fees shall be assessed by the Board:

      1. (A) National Veterinary Technician Examination - at cost
      2. (B) State Examination - $60.00
      3. (C) Application Processing Fee - $50.00
      4. (D) Certificate - $20.00
      5. (E) Annual Renewal - $45.00

   7. (7) Animal Euthanasia technician fees. The following animal euthanasia technician fees shall be assessed by the Board:

      1. (A) Training and Practical Examination - At cost
      2. (B) Oklahoma State Bureau of Investigation criminal history search - At cost
      3. (C) State Written Examination - $60.00
      4. (D) Application Processing Fee - $50.00
      5. (E) Certificate - $20.00
      6. (F) Annual Renewal - $40.00
      7. (G) Reactivation fee - $25.00
2. (b) Submission of fees. All fees are non-refundable.
